CHRONIC BACK PAIN: MORE THAN ONE CAUSE FOR YOUR PAIN?
A correct diagnosis may find more than one cause of chronic back pain. This can be confusing, but patients who have had years of back pain may have a combination of some form of arthritis, pain in the muscles and other soft tissues, and a problem such as osteoporosis.
For these people, successful treatment must attack each separate cause of the back pain. When the basic treatment plan is started, it usually provides a measure of relief and allows the patient to resume daily activity with reasonable comfort.
We recently saw a 55-year-old woman who had had lower back pain for five years. She had gradually limited her activities and was able to walk only a few yards without terrible pain. She remembered no injury.
She was found to have arthritis in the lower (lumbar) spine, inflammation in the muscles and other tissues of the back, and a small fracture of the spine due to osteoporosis.
This is a common combination of problems, and, despite the complications, the treatment is not difficult and usually works well. The woman began a basic treatment program and, over a few months, had significant pain relief. She is now active, participates in a regular exercise program, and has resumed most of her daily activities.
A man in his mid-60s came to see us and said he could no longer ride in a car without severe lower back pain. He didn’t notice this pain while standing, but when he was sitting down the pain became intense. The man loved to travel, and the pain was affecting his retirement activities. Upon examination and with certain tests, he was found to have arthritis in the lower spine. He began the treatment program, and, within just a few weeks, he was out in his car enjoying his travels.
Proper diagnosis allows effective treatment of the problems that cause chronic back pain. Even when your pain has been present over time, don’t let discouragement and fatigue prevent you from receiving treatment that can give relief.
If the treatment you are receiving is directed at the wrong cause of your back pain, you will not have relief. What seems at first to be proper treatment can have delayed or poor results. Remember, if your treatment is not bringing improvement, it is possible that other factors may be causing your pain. Your doc] tor can help you uncover any untreated causes of the back pain and resolve them.
After discussion and examination, one or more of the tests described earlier will be needed. The computed tomographic (CT) scan, magnetic resonance imaging (MRI), myelogram, bone scan, or other tests may be needed to find the causes of your chronic back pain. Possible causes include a ruptured disc, as discussed above, or one of the problems listed in the following sections, or a combination of two or more causes.
