ACUTE BACK PAIN: RUPTURED (HERNIATED) DISC
When acute back pain is severe and travels down one or both legs, the diagnosis may be lumbar disc disease. This is the most common cause of true sciatica.
Marian, a young preschool teacher, came to our clinic with acute back pain that was agonizing. “The pain seems to go all the way out my toes,” she complained. “This all started after I leaned over to pick up a child out of her car seat. My concern is that it is intensifying each day, and I can’t lift any of my preschoolers, much less do anything else strenuous.”
After an exam to determine the problem, Marian was found to have a ruptured disc in her back. In rupture (herniation) of one of the discs between the bones of the lower (lumbar) spine, the disc material causes pressure on a nerve that extends down the leg. The disc rupture may also cause inflammation with swelling around the nerve. The pain is usually severe, as Marian said, and there may also be numbness or tingling. If the intense pain travels down the leg, it may cause weakness of certain muscles of the leg or foot.
As an example, a 40-year-old lineman for a utility company had acute lower back pain when he lifted a heavy load while twisting his body. The pain was severe and traveled down the left leg. He had numbness in the entire left leg and foot and was unable to sleep at night because of the lower back pain. The pain was worse when he sat in a chair and less intense when he stood up.
He found no improvement after treatment. Magnetic resonance imaging (MRI) showed a ruptured disc in the lower lumbar spine. After failure to improve, he had surgery to remove the ruptured disc material. He has since had total relief of pain and has returned to work.
