CHILDHOOD ASTHMA: HOLIDAYS AND STAYING AWAY FROM HOME
In my surgery and in the asthma clinic one of the factors that causes great concern to parents is whether it is safe to go on holiday. My answer is invariably ‘yes’, as it is important for a child with asthma to lead as normal a life as possible. I have a young teenage patient who is going to Kenya on her third safari this year! The only time I would recommend staying at home is if the asthma is in an unstable condition. If attacks are coming very frequently it is wise to have the treatment reviewed and appropriate action taken in the stable environment of your own home. Usually this does not take long and hopefully only means a postponement of the holiday rather than a total cancellation. For the vast majority of asthmatics it is perfectly safe to go away but as a precaution you must ensure that you have the means to deal with an attack if one should occur.
Most vital is to remember to take the medication with you! This may sound silly but for three years I worked in a hospital in Blackpool and we once calculated that over half our admissions in summer were people who had forgotten to bring their medicine with them. Inhalers are usually the last to be packed so they can be easily reached and it is amazing how often they are left behind on the bedroom table. Also make sure you have one or two spare inhalers with you as they do have an irritating habit of running out or being lost just when needed most. If you are staying in this country then an attack can almost be as easy to treat as at home. It is well worth remembering to check when you arrive at your destination as to where the local doctor is based and how to make contact in an emergency. Everyone in the United Kingdom is entitled to medical treatment wherever they are staying free on the N.H.S. If you are staying in a hotel – and this applies particularly in London – the staff will often call a doctor privately for you, and the visit plus treatment can take up most of your holiday spending money. You are within your rights to ask for an N.H.S. visit and you should always insist on this. A useful tip is to take your own doctor’s phone number away with you and give him or her a ring first for advice. It is not very often necessary to call a doctor out, especially if you follow the treatment plan.
The first night of a holiday is the one when your child is most likely to suffer an asthma attack as the change of environment throws up different kinds of allergens and also there is all the excitement. So be organized and unpack the medication before you retire to bed.
If you are travelling abroad the situation is slightly more complicated, though if you are well prepared you should have little trouble. First, before you go, make sure you have adequate medical insurance just in case you need it. If you are going to the United States also ensure you have a credit card with sufficient reserve funds on it, as often the hospitals over there want the money before they will give the treatment. The last thing you want at that time is to have the stress of ringing insurance companies to obtain a cash advance.

COMMON SKIN DISORDERS IN ADULTS: ADULT ACNE IN WOMEN

Skin disorders are quite common in adults. Because they are often visible, on the face or hands for example, sufferers may be very self-conscious or even embarrassed about them. Nearly all of the population gets some sort of skin, hair or nail disorder at some time in their lives. Most of these are not serious and certainly most are treatable. They are not things you must simply ‘learn to live with’.
Adult acne is becoming increasingly common in women, with many continuing to have blemishes into their thirties and forties. The exact cause for this is not known, although there are several possibilities.
Some women have true ‘hormonal acne’ which is associated with premenstrual pimples and irregular periods, and occasionally with excessive facial hair. Hormonal acne responds well to Aldactone and Androcur. These drugs work against excessive production of male-type hormones.
In other women acne is caused by excessive use of moisturizing cosmetics. Usually, women in their twenties start becoming aware of skin care products, and spend their hard-earned pennies on various cleansers, moisturizers, anti-age creams and night creams, only to find that blemishes appear for the first time. Once they stop using such creams their skin tends to improve dramatically. For these women, correct skin care advice (away from department stores) is essential.
Stress may be another contributing factor. Many women today find themselves working not only in paid employment, but bringing up children and running a household as well. Stress is thus an escalating factor in most women’s lives and it is not surprising that acne can be a manifestation of this.

This is the name given to an inflammation of the skin that is characterized by branny scales. There are many different forms of it, all receiving special names, but it is clearly a symptom that needs very little direct treatment. Psoriasis
This is a chronic inflammatory disease of the skin marked by the formation of scaly red patches. It begins with tiny scale-like lesions which join together, and in bad cases may disfigure almost the whole body. Like so many other skin complaints, it may vary with each individual and change from time to time in the individual himself. It appears to run in families, and the hereditary factor should be taken into account because it enables one to endeavor to make as complete a change in surroundings as possible. This is a difficult condition to overcome, and this fact has tended to develop a pessimistic outlook, both in the patient and in the attendant, which stands in the way of recovery. All the factors making for physical, mental and emotional balance will have to be carefully adjusted to obtain the desired results.

DISEASES OF THE SKIN: FAVUS, HERPES AND ICHTHYOSIS
Favus
This is sometimes spoken of as crusted ringworm. It is due to a parasite and is regarded as contagious. The susceptible skins are those that have lost their natural immunity; not all members of a community will get this complaint, which shows that there is a natural barrier to it. Individuals who suffer in this way are usually of poor nutrition, and urgent body-building measures are required in addition to the local treatment. It is characterized by a small saucer-shaped yellowish crust that forms around the hair follicles and may give off a musty odour.
Herpes
Everybody is familiar with this eruption on the skin. It appears as a little group of vesicles that form but give no particular trouble. They are generally connected with an elevation of the bodily temperature, and often follow feverish colds, appearing around about the lips. These are sometimes called cold sores. Their appearance in children often indicates that there has been an unsuspected temperature. They clear up as the general condition of the body improves.
Ichthyosis
This is sometimes called fish-skin and is a difficult condition to overcome. Some authorities are inclined to the view that it may be hereditary, but that is no reason for instituting despair in the patient. It is true that external applications are not of much avail, but a systematic health-building regimen should be adopted and carried out with great persistence. If neglected, the surface of the skin may become very hardened and wart-like growths may appear.

Facial Veins
Facial spider veins may suddenly appear during pregnancy, usually on very conspicuous parts of the face such as the nose and cheeks. They are caused by an increase in oestrogen hormones. They may also occur when the oral contraceptive pill is taken. They are not caused by exposure to hot or cold weather and are not due to drinking coffee. Many of these spider veins do not disappear after pregnancy and may require treatment with laser or fine needle diathermy.
Spider Leg Veins
Disfiguring spider leg veins, which are mostly hereditary, often become established during pregnancy. They are not caused by excessive standing or sitting with the legs crossed. It seems that a combination of hereditary and hormonal factors stimulate growth of these vessels. Once they appear, they rarely disappear. Spider leg veins can be readily treated by a technique called microsclerotherapy. A concentrated salt solution or polidocanol is injected into the veins, which initially become inflamed and then disappear over the following six weeks. The technique causes very little pain and can be done as an outpatient procedure.
Varicose Veins
Varicose veins may also make their debut during pregnancy due to the combination of hormonal changes and pressure placed on the abdominal veins by the growing uterus. Because varicose veins can be extremely uncomfortable, maternity compression stockings are invaluable. Support pantyhose, on the other hand, are counterproductive as they provide irregular compression, usually on the wrong parts of the veins. If varicose veins do not resolve after pregnancy, they may eventually be treated by either surgery or sclerotherapy. It is preferable to wait until all pregnancies are over before embarking on any surgical treatment, although sclerotherapy can be performed at any time after pregnancy and between pregnancies.

DISEASES OF THE SKIN: ECZEMA AND ERYTHEMA
Eczema
In this complaint there is inflammation of the skin, with the formation of pustules, scales and crusts. It may be dry, or it may keep up a watery discharge. It may be found in persons of all ages, and is particularly troublesome when it attacks very young infants because of the intense irritation. It may be of the acute or the chronic kind, and it requires patience and persistence to get rid of it.
The name, which is taken from the Greek, is interesting because it means “to boil out,” and this is a very good description of what goes on. There is no point in trying to suppress it; it has to be eliminated by the most careful adjustment to the environment and by treatment that will build better health. It would take a volume to describe the many forms through which the complaint may go, but this is of little consequence so far as the Nature Cure point of view is concerned. It is not a disease but a constantly varying symptom.
Erythema
This is a term that is often used in connection with redness of the skin, involving the cutaneous circulation. It is generally supposed to be associated with the nervous mechanism of the system, or it may arise from contact with some external agency, such as, for example, the direct rays of the sun. It is not infrequently seen as a symptom of certain diseases, and a redness of this order, erythema nodosum, often accompanies rheumatic diseases. Drugs, also, may be the cause of this skin reaction. As with dermatitis it may take on many different forms, but there is no need here to enter into the names and descriptions of them.

SKIN DURING PREGNANCY: ACNE
One of the most common myths is that acne will disappear during pregnancy. Although this does occur in some women, others notice that their acne gets worse or appears for the first time due to sudden hormonal changes.
Unfortunately, treatment options are very limited because many drugs used for acne are not safe during pregnancy. Simple measures are therefore very important. Moisturizers and moisturizing sunscreens should be avoided, and oil free makeup should be used.
Topical antibiotics, such as clindamycin lotion, are safe during pregnancy, as are benzoyl peroxide and topical sulphur creams. Retin-A can also be used during pregnancy as it is not sufficiently absorbed into the circulation to cause any problems. Certain oral antibiotics such as erythromycin are safe while the tetracycline should not be used. The latter can cause problems in the bony growth of the foetus during the second trimester. If tetracycline is accidentally taken in the first trimester there are no untoward consequences.
Ro-Accutane must never be used during pregnancy as it can cause major birth defects. If a woman has taken Ro-Accutane she should not get pregnant for at least one month after she has stopped taking the drug.
Acne may worsen during breast-feeding due to the substantial oestrogen level decrease. During this period, all topical anti-acne preparations can be used. Oral erythromycin is also safe, but other oral preparations should be avoided.

SKIN DURING PREGNANCY: PIGMENTATION
Pregnancy is often considered a time when the complexion blooms, when the skin looks better than ever. Although this is true for many women, there are others who begin to develop skin blemishes at this time. Moreover, women may develop stretch marks, varicose veins and spider leg veins, and suffer from very itchy skin and cracked nipples during pregnancy. There are preventative measures which can help women avoid these problems, and most of them can also be treated once they have occurred.
Disfiguring pigmentation, especially of the cheeks and upper lip, commonly occurs in pregnancy and when the oral contraceptive pill is taken. This is caused by a combination of oestrogen hormones and ultraviolet A light, which penetrates windows and most conventional sunscreens. The pigmentation may disappear spontaneously after pregnancy, but in most cases it will lighten during the winter months and darken again in summer.
Because ultraviolet A light is not blocked adequately by conventional sunscreens, it is difficult to prevent this pigmentation. It is best to keep out of the sun as much as possible during pregnancy. Outdoors, a sunscreen such as tinted zinc cream, Clinique Continuous Coverage, Dermablend and pancake make-up is the best choices. The newer sunscreens which contain titanium dioxide and parsol 1789 (methoxy-dibenzoyl methane) give good but incomplete protection from ultraviolet A light. These include Ego SunSense, Ego Sun-sensitive, Clinique City Block, Nivea Visage with sunscreen and UV Low Allergenic Formula.
Pigmentation may fade after pregnancy if sun exposure is diligently avoided. Otherwise, it is extremely difficult to treat. Fading creams available from chemists are ineffective. However, a prescription cream containing 5% hydroquinone and retinoic acid may help to fade the pigmentation in some women. This must only be used under a doctor’s supervision after pregnancy. The depigmenting lotion does not bleach the skin white, but rather fades it to its normal colour. Initially it produces some redness and peeling until acclimatization occurs. In severe cases chemical peeling may be useful but is not without risks, and must be done by a suitably qualified doctor.

ACNE IN ADOLESCENCE: BEAUTY THERAPIES, ACNE SCARRING AND OPEN PORES
Beauty therapies
There are many beauty therapies available for acne, most of which have no scientific rationale. While there is definitely a place for blackhead and whitehead extraction by beauticians because this unblocks the oily glands, it does not remove pusy sores. Other treatments such as cold lasers, acne creams, facial scrubs and masks have no proven benefit.
Acne scarring
Hopefully, in the twenty-first century, people will no longer have to live with acne scarring. Prevention of scarring is most important, as the treatments available for acne scarring are not entirely satisfactory.
With time many acne scars do improve without treatment. For more severe scarring there are several treatment options. Lumpy acne scars may be injected with cortisone to make them flatter. More commonly, however, acne scars are depressions in the skin which may be valley type or very sharp pits. Valley-type scars may be injected with collagen or silicone. This is generally done by dermatologists. Although collagen is very popular, it needs to be injected every one to two years. Allergies to collagen may also develop, although they are rare. Silicone, on the other hand, is permanent. It must, however, be injected very carefully in minute quantities; the correction is permanent so there is no room for error. Although silicone has received a lot of bad press recently, it has been used for over thirty years to improve facial deformities and causes few problems if used in expert hands.
Pitted acne scars are best treated with dermabrasion or laser abrasion, which is done by dermatologists and plastic surgeons. During this procedure the skin is ‘sandpapered’ to make it smoother. Although dermabrasion is an effective procedure for acne scarring it does have limitations, in that it can never make the skin entirely perfect, especially if scarring is deep. Dermabrasion is not without risk and can produce scarring and changes in the skin’s pigment. Chemical peeling is often advocated for acne scarring but is not as effective as dermabrasion.
It is best to wait at least twelve months after acne clears before having any acne scarring treatment. Much of the scarring will resolve in this time, during which cover-up cosmetics can be applied.
Open pores
Open pores are often evident on the nose in people with oily skin. This is a hereditary condition which cannot be altered by any cream or cosmetic. Oily secretions may accumulate in open pores, causing blackheads, which can be removed by manual extraction or with the use of Retin-A cream.
Although there are products whose manufacturers claim can open and close open pores, this is medically impossible. Regular use of Retin-A cream will, however, minimize the appearance of open pores. Oil-absorbing lotions such as Almay oil absorbing lotion or Nutrasorb will also make them appear less shiny and so less conspicuous. Sally came to see me when she was forty-one because she was determined to do all she could to prevent the symptoms of the menopause. The biggest breakthrough in the treatment of acne has been the introduction of Ro-Accutane, which is a vitamin A derivative taken in tablet form. It was discovered by accident that this drug penetrates into the sebaceous glands and decreases sebum production. Vitamin A itself in the doses required is far too toxic and is ineffective for acne.
Ro-Accutane is the most effective treatment for acne but is used mainly in more severe cases. It often cures acne completely. Loading the device
1. Remove the mouthpiece cover and ensure the mouthpiece is clean.
2. Hold the corners of the white tray and pull out gently until the ridged areas on the sides of the tray are fully visible.
3. Squeeze the ridged areas and pull the tray, with the wheel, out from the diskhaler body.
4. Do not remove the wheel. Place the disk on the wheel with numbers uppermost and then slide the tray back fully into the body of the diskhaler.
5. Rotate the disk by moving the tray out and in gently, until the number 8 appears in the indicator hole. As the device is used this indicator hole shows the number of doses remaining.
Using the device
1. Hold the diskhaler firmly in a level position. Lift the rear edge of the lid and raise it as far as it will go until it is fully upright, thus puncturing the blister. Some resistance will be felt as the upper and especially the lower surfaces of the blister are pierced.
2. Close the lid.
3. Keeping the inhaler level, breathe out fully.
4. Raise the diskhaler to the mouth and gently place the mouthpiece between the lips without biting. Do not cover the air inlets on either side of the mouthpiece.
5. Breathe in through your mouth as quickly and as deeply as you can.
6. Hold your breath briefly to allow the powder to reach the lungs and then remove the diskhaler from your mouth.
7. Slide the tray out and in once so that the next number appears in the indicator hole. Do no pierce the blister until immediately before inhalation.
Each disk consists of eight blisters containing medication. When the number 8 reappears in the indicator hole, all eight doses have been taken and the disk should be replaced by repeating the procedure above. Please make sure you do not throw the wheel away with the empty disk. All this sounds very complicated but in practice is simple and does have the advantage over the pressurized inhalers of not requiring coordination of breathing in while pressing the canister. The major disadvantage is that as each disk only contains eight doses it is necessary to carry spare disks and often children forget. I have also found they do tend to clog up with powder. The manufacturers do provide a brush in the rear of the diskhaler body to clean any remaining powder. This should be carried out with the tray and wheel removed from the body of the diskhaler but again in my experience children cannot be bothered to do this, and the efficiency of the device is gradually reduced.

Local applications to the unhealthy skin
As a general rule one should never rely too much on local treatment, no matter how circumscribed the skin ailment appears to be. But there are cases where local measures must be employed to help to increase the local circulation, to break up congested spots, to relieve irritation, and to give comfort in painful ailments. For these purposes we may make good use of the various compresses and hot fomentations which assist in elimination and are never harmful. If pain and discomfort can be relieved in this way it is so much better than using pain-killing drugs which invariably upset the digestive system.
The evaporating compress
When there is an overheated and painful area of the skin great relief may be obtained by the use of the simple evaporating compress.
The part is covered by a wet cloth, which still retains as much ordinary cold water as possible, and should not be covered by any other material. The heat of the skin will soon raise the temperature of the compress, and then it should either be quickly changed and another cold one applied, or cold water should be poured over the one already in use. The idea is that the skin must be kept moist all the time, and the evaporating process gives great comfort and relief.
The neutral compress
In some cases the neutral compress is a very useful application that will give more relief to some people than will the cold one.
The temperature of the compress should be about that of the normal skin, and used in this way it is soothing, especially to the great system of nerves under the skin. It should be employed in all cases where the skin troubles seem to set up nervous irritations, and in those cases where the pain is of a neuralgic character it is well worth trying. It excludes the air from contact with the skin and at the same time keeps up a uniform temperature in all the tissues. By keeping the skin moist it softens the parts and allows the skin to absorb a certain amount of water into the external parts. When the skin is affected by a complaint that is clearly associated with an irritated nervous system this compress should be freely employed for its soothing qualities.

SKIN IN ADOLESCENCE: HORMONE TREATMENT FOR ACNE
Hormone treatment is only useful for women with acne. This treatment is based on the fact that acne often starts at puberty and may worsen just prior to a woman’s period. It may improve during pregnancy and when taking a high oestrogen, low progestogen contraceptive pill such as Biphasil or Sequilar.
Other hormone treatments include taking Androcur and Aldactone tablets which block the male-type hormones which women produce just before a period. These drugs have minimal side effects but do cause irregular periods, so should be combined with the pill or an oestrogen hormone.
Many women are averse to taking hormone treatment for fear of gaining weight. Although the pill may cause slight fluid retention, significant weight gain will only occur if excessive calories are eaten. Many people blame the pill for poor eating habits, which are the real cause of weight gain. The other fear of hormone treatment is that it will lead to infertility later in life. Although the pill does suppress ovulation, this is only temporary and will not cause long-term problems.

SKIN IN ADOLESCENCE: ORAL THERAPY FOR ACNE
Most people with moderately severe acne require oral treatment in combination with creams. The main forms of oral therapy include antibiotics, hormone treatment and Ro-Accutane (Isotretinoin).
Oral antibiotics
Until recently oral antibiotics were the mainstay of treatment for moderate to severe acne. Generally speaking, they are very successful in treating acne, and have minimal side effects.
One common myth about antibiotics is that they lead to thrush in the bowel, which leads to chronic illness. Although antibiotics do increase the incidence of vaginal thrush, this is only a temporary phenomenon. They do not cause long-term illnesses. It is also sometimes believed that if antibiotics are taken for acne they will not work for more serious infections, which is not the case.
The most commonly used antibiotics include tetracycline, minomycin, erythromycin and Bactrim. Usually, antibiotics need to be taken for a period of at least three months before any benefit is seen, and they need to be used on a prolonged basis as they do not ‘cure’ acne. The mainstay of therapy with antibiotics is therefore to suppress the acne until it spontaneously improves. The main limitation of antibiotics is that they are not effective in cases of severe cystic acne and become less effective with time, as resistance occurs.
Most people tolerate antibiotics very well but may be fearful of taking them for a long time, despite their safety and effectiveness. (Exceptions here are the tetracyclines, which should not be taken by children under eight as they can stain the teeth and accumulate in developing bones. Likewise, although Erythromycin is safe during pregnancy, other antibiotics should not be taken during this time.)

SKIN AND DISEASE: ACTIVATING SKIN WITH THE EPSOM SALTS BATH
A very useful way of stimulating a sluggish skin is the use of the Epsom salts bath. The Epsom salts may be bought in large quantities, seven pounds or more, and should be of the crude kind for bathing purposes. From one to two pounds should be dissolved in a bath of hot water, and no soap or anything else added. The temperature will depend, of course, on the general condition of the patient; but one might say that it should be as hot as can be reasonably borne, because it should last for about twenty minutes and by the end of that time it will have cooled considerably.
In order to make this bath really effective the superficial circulation through the skin tissues should be stimulated. This may be done by the simple method of lifting the skin under the fingers and thumbs. Starting with the feet and legs, one should work all over the body, trying to pick up the skin in folds. It is, of course, much easier to do this in certain parts of the body?on the abdomen, for example; but no part of the body should be missed. By going systematically all over the body the circulation can be greatly increased and local congested spots broken up.
If there are actual sores or irritation on the surface of the body these parts should not be directly treated. Instead, one should work all around them, trying, as it were, to stimulate the near-by circulation. This is a most useful procedure. It is helpful in the case of boils, assisting the lymph to carry away the offending waste products.
If the lifting of the skin is done properly the individual will experience a glow all over the body and then, when he relaxes, as he should, before getting out of the bath, he will be conscious of a tingling movement throughout the whole of the surface of the body. One might say that this measure has stirred up the “sea-water” in the skin region and hastened the waste products on their way to the depurating organs. The effect of this will be felt, not only on the health of the skin, but on the health of the whole body. In fact there is no better measure in many chronic diseases than naturally to stir up the effete matter that is in the system and eliminate it through the proper channels.
If the patient is possessed of a fair amount of energy this bath may also be the time for doing a few exercises. With the body immersed in the water there is no better time for limbering up the muscles and joints, and exercise done in this way places very much less strain on the heart and lungs. It is possible in this position to stretch the limbs and to contract many of the groups of muscles, thus aiding the circulation and improving muscular tone.
A word of caution should be uttered here. The bath must not lead to exhaustion, and if the patient is at all depleted in energy it may be a good plan to do the exercise one day and the skin lifting movements on another. But all who suffer from a sluggish skin and all the ailments that go with it should use these measures, because they will do more than anything else to stimulate the circulation and reactivate the skin.
Some people find sea salt more pleasant and effective than Epsom salts, and it may be a good plan to alternate them. Apart from these two substances there is no need to make any use of medicinal preparations.
