PROTECTING YOURSELF AGAINST COMMON POISONS: STRONTIUM 90

World-wide nuclear tests and explosions have now contaminated the whole globe with toxic Strontium 90. Scientists say that everyone has already dangerous amounts of radioactive Strontium 90 in their bones. It stays in the body throughout the lifetime, emitting radioactive rays, like x-rays.
Anemia, leukemia, sarcoma of the bones (bone cancer) and many other cancers are believed to be caused by Strontium 90.

Protection
1. Algin. Extracted from giant brown Pacific kelp, this non-toxic substance can effectively remove radioactive Strontium 90 from the body. Algin can be used in the daily diet instead of such thickening agents as gelatin or cornstarch, or mixed with milk or other drinks.
2. Pectin. It binds radioactive Strontium in the intestines and reduces its absorption and deposition in the skeleton. Only the pectin derived from sunflowers was found effective. Eat plenty of raw sunflower seeds daily.
3. Calcium and magnesium. Both help your body to pass off Strontium 90. When selecting your mineral supplement, make sure it is not made from American animal bones which contain large amounts of Strontium. The best bone meal supplements are made from South American bones, which are not so contaminated. Bone meal with bone-marrow is the best. Deep-mined mineral supplements, such as dolomite and others, are advisable. Dr. Linus Pauling says that heavy calcium supplementation will reduce strontium absorption by 50 percent.
4. Yogurt and other soured milks help to neutralize the radioactive chemicals in the intestines and excrete them safety from the body. Up to 1 quart a day of soured milk can be consumed.
5. B-complex vitamins, or Brewer’s yeast. It has been shown in animal studies that brewer’s yeast affords protection against radiation.
6. Kelp. Sodium alginate in kelp reduces absorption of Strontium 90 by 50 to 80 percent. Take 1 to 2 tsp. of granules daily, or 5 to 10 tablets.
7. Lecithin. Lecithin in daily diet can counteract the effects of radiation.
8. The following vitamins can help to guard against radiation toxicity: E, C, and B-complex.

PSYCHOLOGICAL AND PHYSICAL REST

There have been few surgical classics. Mr. Hilton, of Guy’s Hospital, London, England, wrote one about a century ago on The Therapeutic Influence of Rest and the Diagnostic Value of Pain. He pictured the expulsion from the Garden of Eden, the first wound, and resulting dismay; and, “the original promptings of nature to man for the alleviation of his altered condition. Pain was made the prime agent. Under injury, pain suggested the necessity of, and, indeed, compelled him to seek for, rest.”
His thesis was that nature could heal the injuries caused by wounds or disease only when the affected part was allowed to rest. And by rest he did not necessarily mean lying in bed or sitting in a chair. He spoke of physiological rest. A piece of dirt lodges under the eyelid and soon there is a conjunctivitis. Nature cannot cure this as long as there is no rest for the tissues because of irritation. Remove the dirt and rest will soon effect a cure even without silver nitrate.
Compound fractures in recent years have been treated by good cleaning, and putting up the affected parts in plaster casts where they cannot be disturbed by dressings or movements. This does not mean that the patient cannot tire himself out by traveling about on crutches. The patient with appendicitis gets physiological rest when the sick organ is taken out. Even a male can appreciate that a woman who was pregnant has achieved physiological rest when her constant companion of nine months is safely in the nursery.
On the other hand sometimes physical rest is needed. A physician caring for a rheumatic fever case may feel that he is putting extra work on the heart by allowing the patient to move about. He may doubt that this is compensated for by the aid to circulation that muscular activity gives. The liver is the largest organ of the body with a multitude of chores to do. Extra activity on the patient’s part undoubtedly increases these chores. So Dr. Chester Jones, of the Massachusetts General Hospital, speaking to us a few years ago on liver diseases, stressed, next to diet, the importance of physical rest in treating these conditions. At about the same time Dr. Thorndike, of the same institution, depicted to us the dangers of staying quietly in bed. His patients were started walking within twenty-four hours of abdominal operations, women were up and about forty-eight hours after childbirth, and even people with heart trouble were let out of bed promptly.
How do we explain these apparently divergent views emanating from this shrine of Aesculapius? First we must remember the human propensity to swing upon a pendulum, and that in nearly all matters we go in cycles from one extreme to another. A short generation ago obstetrical patients spent several weeks of convalescence in bed, especially if they were well-to-do and could afford the care. The modern shortage of hospital beds and the general enthusiasm for acceleration programs have tended to demonstrate that this is not at all necessary. But analysis may show that the two methods are not so divergent in results when considered from Mr. Hilton’s point of view.
Mr. Hilton had a patient who broke his leg and when put to bed developed jaundice. So Mr. Hilton reasoned as follows, I believe the congested liver which leads to the jaundice results from the forced rest to which the liver is subjected by the recumbent position; the circulation through the organ up to the period of the accident having been aided by active respiration and ordinary exercise. The withdrawal of these leads to congestion of the liver and hence jaundice.” It may well be that when a patient is too inactive and the circulation slows down, actual physiological rest is thus lost.
We all believe in physiological rest. In each case we must decide whether complete bodily rest will give this. As Hippocrates said in his first aphorism, “Decision is difficult.”
Although we will all accept rest as beneficent – and possibly my arguments may have produced a grumbling acceptance of pain – nevertheless it is with diffidence that I now ask you to include inflammation in this blessed category of protective devices. The word is, of course, closely related to inflammable, and refers to what Celsus, a hundred years after Christ, called the first of the four cardinal signs of inflammation: calor, rubor, tumor, and dolor (heat, redness, swelling, and pain).

Whatever you call it, you are not alone. Scientists at Johns Hopkins University in Baltimore conducted a study to find out how many people were mentally ill. They discovered that about one third of the 3,481 persons surveyed said they had at some time sought help for a mental or emotional problem (or a drug or alcohol problem). They found help by talking to a friend, a relative, a mutual help group, or professional.
The data also indicate, says Dr. Ernest Gruenberg of the Baltimore study, that one person in nine said he or she feels the need for professional help now.
Many problems come from an overload of stress. You lose your job, for example, or your spouse to death or divorce, or someone you love becomes seriously ill. A new baby is born, or a new job means moving and starting over in another town. You feel troubled, inadequate and unable to cope.
Not everyone who feels bad has overwhelming problems. Some people suffer from irrational fears, or phobias. They can’t enter an elevator or a tunnel, for example, without panicking. When the reaction to stress or anxiety is severe, it can lead to nervous collapse or to alcohol and drug abuse.

After puberty, the sex hormones are busy, and many secondary sex characteristics are due to them. Boys and girls before puberty are much alike. Then fat begins to fill in the furrows of the female, giving the pleasing curves; the pelvis broadens and the breasts fill out. Some of the distinctions between the two sexes at this time are due not to changes in the female but to the fact that the male changes and the female does not. In early life the larynx grows in about the same manner in boys and girls. That is why boys can sing soprano. At puberty it increases in size, relatively more in males than females, and especially from front to back. Then the lengthened vocal cords give deeper notes in men than in women. If a boy is castrated well before puberty, the larynx may not enlarge and the voice may remain high pitched. It is said that through the eighteenth century this was sometimes done to keep an exceptional soprano. Terrible things have been done in the name of art. Up to puberty girls and boys both have soft skins and peaches-and-cream complexions. Males in general seem content to take the changes as they come. I have seen no advertisements of male hormone preparations to hasten the appearance of manly beards. Women are different. Commercial enterprise is persuading many young women to use sex hormones. The glorification of the female breast solely because of its pulchritude and aesthetic quality has been seized upon by rather unscrupulous business. Hormonal creams are being urged to keep the skin soft and youthful. Not only are these preparations expensive and of exceedingly little value, but if they ever do succeed in getting into the system there is a good chance of upsetting a very delicate balance.
How valuable are the sex hormones in treatment? I have talked with gynecologists, genito-urinary specialists, and the head of one of the highest-grade drugstores in the country and have found, as was to be expected with the modern emphasis on sex, and the irresistible power of advertising and publicity, that they are sold in enormous quantities. Possibly less is known accurately about them than about any of the other hormones.
Adolescence, the period of developing maturity just after puberty, is a time of rearrangement of hormones or internal secretions. Although some of the disturbances are physical, the greater proportion is psychic. A rough-and-ready member of our profession, now dead, gave a striking example when he told of a young girl whose parents had been advised by a specialist to give a hormone for her adolescent troubles. He carefully removed the seal from a bottle of pellets and substituted similarly shaped pills of cooking soda. He got a prompt and excellent cure.
Our laws have fixed a definite upper limit of adolescence. Our hormones conform to this law much more often than our mentalities. Gilbert and Sullivan knew of a perfect world where the fairy, Iolanthe, was still seventeen although her half-human son was already five-and-twenty. However, the young lady would not be allowed to vote in either Britain or America for it is the chronological, not mental, age which determines the right to suffrage.
Although both male and female reproductive organs are developed at puberty, adolescence, during which the sex hormones mature, is the period before the adult man and woman are really prepared, psychically as well as physiologically, for that vital function, reproduction.

Functions
Of all vitamins, vitamin B12 is the most complex. The trace mineral cobalt is an essential part of the molecule. Vitamin B12 is required for the maturation of the red blood cells in the bone marrow; for the synthesis of proteins; and for the metabolism of nervous tissue.
Absorption and storage
The absorption of vitamin B12 is much more complex than that for other B-complex vitamins. The stomach produces a protein-carbohydrate substance called intrinsic factor. Vitamin B12 is attached to intrinsic factor and is carried to the ileum from which it is absorbed. In the absence of intrinsic factor vitamin B12 cannot be absorbed.
The liver stores most of the vitamin B12. On an adequate diet, the storage in the adult may be sufficient to provide the body’s needs for as much as 3 to 5 years.
Meeting daily needs
For adults 3 mcg vitamin B12 is recommended daily; 0.5 to 1.5 mcg for infants, 2 to 3 mcg for children, and 4 mcg during pregnancy and lactation.
Milk, eggs, cheese, meat, fish, and poultry supply ample amounts of vitamin B12 even though the intakes of these animal foods may be relatively low. Plant foods supply no vitamin B12 and use of an exclusively vegetarian diet for a long period of time will lead to symptoms of deficiency.
Clinical problems
Pernicious anemia is the disease resulting from vitamin B12 deficiency. It is a genetic defect with an absence of intrinsic factor; hence the vitamin B12 in the diet cannot be absorbed. Patients with pernicious anemia have a macrocytic type of anemia; that is, the red blood cells are large and reduced in number. There are frequent complaints of sore mouth, poor appetite, and gastrointestinal disturbances. The nervous system is affected so that the individual shows poor coordination in walking, for example; his mental processes may also be affected. Parenteral injections of vitamin B12 effectively control the condition; they must be used throughout the life of the patient.
Patients who have been subjected to surgical removal of the stomach also exhibit signs of vitamin B12 deficiency after a period of months or years if they are not given parenteral injections of vitamin B12. If children are given an exclusive plant diet, there is slowing of growth; this is the only situation in which dietary deficiency of vitamin B12 occurs.

DOSE OF EVENING PRIMROSE OIL FOR HYPERACTIVE CHILDREN
Age under two years: please contact HACSG as it is necessary to work out individual requirements carefully.
Age two to five years: two evening primrose oil capsules of 500mg a day, rubbed into the skin or taken by mouth. Plus multi vitamin and mineral tablet, plus co-factors: two tablets per day Vitamin 250 mg, nicotinamide 15 mg, Vitamin B6 50 mg, zinc sulphate 5 mg.
Age six to seven years: three evening primrose oil capsules of 500 mg a day, either taken by mouth or rubbed into the skin. Plus a multi vitamin/mineral tablet, plus co-factors: three tablets per day of Vitamin 375mg, nicotinamide 22.5mg, Vitamin B6 75mg, zinc sulphate 7.5mg.
Age seven upwards: four evening primrose oil capsules of 500mg per day, either taken by mouth or rubbed into the skin. Plus a multi vitamin/mineral tablet, plus co-factors: four tablets Vitamin 500 mg, nicotinamide 30mg, Vitamin B6 l00 mg, zinc sulphate l0 mg. Dose can gradually be increased to six evening primrose oil capsules. From the ages of eight or nine upwards, five or six capsules of 500mg evening primrose oil are effective. Begin with small amounts and gradually increase.
For each age group, take the tablets twice a day, once in the morning and once in the evening with food. If you rub the oil into the skin rather than have the child take it by mouth, choose a soft part of the body such as the abdomen, top of thighs, or inside forearms. The oil is very fine and can be rubbed in until it is completely absorbed. The gelatin shell of the capsules can easily be punctured by a safety pin, and the oil can then be squeezed out into the palm of your hand.
Note: If swallowing the oil or rubbing it in is a problem, try adding it to butter or margarine so the child does not notice it.

CONTROVERSIES IN REFRACTIVE EYE CARE
Next to the brain, the eye is the most complex organ in the human body. Like most any delicate and enormously intricate human body mechanism, unfortunately, the eye does not always function properly. Almost half a million people in the United States are legally blind and many others suffer from a wide range of visual disorders. They are especially subjected to refractive problems including myopia, hyperopia, presbyopia, and/or astigmatism.
Over the past decade, dramatic breakthroughs have been made in medical scientists* understanding of the eye. Vision experts are attaining more knowledge concerning the cause of eye difficulties, new scientific instruments to help detect eye trouble have been developed, and meaningful breakthroughs entailing surgical techniques to cure visual disorders have taken place. Yet, with all the breakthroughs in a diversity of eye care fields, controversy among people rendering professional health care services has arisen. Competition is keen. Sometimes the many contentions, particularly between optometrists and ophthalmologists or between optometrists and opticians or among opposing groups within the separate professions themselves are bitter and combative.
Disagreements among these eye care professionals, in particular those who treat refractive problems, are based upon the von Helmholtz theory. About 126 years ago Herman Ludwig Ferdinand von Helmholtz, M.D., the inventor of the ophthalmoscope, came up with the concept of accommodation on which orthodox ophthalmology and optometry are founded. Dr. von Helmholtz claimed that accommodation is effected by the change in shape of the lens. In turn this change is governed by the action of the ciliary muscles, although he did not offer any reasonable explanation as to how the ciliary muscles operated. He also admitted that his theory was merely a probability because the image obtained on the lens was so variable and uncertain that to use his own words, it is “most usually so blurred that the form of the flame could not be definitely distinguished”.
Dr. von Helmholtz declared that nearsightedness and farsightedness, as well as most other errors of refraction, were fixed states. He stated unequivocally that these conditions could not be corrected. He believed that somewhere along the line either through birth or another reason these faults existed and there just was no cure for the situation. CHRONIC CONFUSION: PARKINSON’S DISEASE
This is a condition that most people have heard of but very few know about. It was first described by James Parkinson in 1817 and his description of his first six cases has never been bettered.
He worked as a doctor in Shoreditch in the East End of London, and whilst out in the market one day he saw two men, both were running but the front one was upright and the one behind severely stooped and only kept from falling by the front man’s hand under his chin. He watched them run daily through the market area, and gradually got to know them. They were brothers, and as one became more bent over he could only stagger forwards, getting raster and faster, and would fall over without his brother’s help. Parkinson made a study of the stooped brother and over the years round five other cases that he wrote up for the medical journals, describing them as having the ‘shaking palsy’. His detailed assessment of the condition, later to be known as Parkinson’s disease, missed very few symptoms and signs. Parkinson did however state that he thought that in this condition the intellect was spared. This is now being questioned.
Parkinson’s disease is one of the most common neurological disorders of the elderly. It has many special features. One is the tremor of the fingers and hand. This usually starts on one side and is very rhythmical; the fingers are said to resemble someone who is ‘pill rolling’. There is difficulty in starting a movement so that getting out of a chair or bed can be very difficult. The muscles become more stiff and rigid so that movements are slow and the face becomes blank and staring as the facial muscles become affected. All movements become disordered and the person tends to fall easily because they cannot balance well, and as the disease progresses they tend to stoop forwards and hence fall forwards. To try and prevent overbalancing the person takes little fast steps and may find themselves at a trot like Parkinson’s first case.
The natural arm-swing goes, so the arms hang by the side and even the muscles in the gullet can be affected, making swallowing difficult. The skin can become very greasy with a tendency to spots, and the bladder can be affected, causing extreme urgency to pass urine and often incontinence. All these features may not occur at all in some people when the disease is very mild; for others the symptoms and signs described come on over years but can cause increasing severe disability – all aspects of daily life can be affected, even down to handwriting, so that letters and cheque signing become impossible and buttons and laces something to avoid.
It is not surprising that depression is very common in this condition. The expressionless face and slow, often whispered, speech can lead people to think that the person behind the mask is simple. As we shall see, chronic confusion can be a feature of the condition but for most sufferers their mind is alert, seemingly trapped in a body that won’t do what they want it to. The greasy skin and tendency to drool at the mouth adds to the distress and misery. Depression should always be looked for because it is, amenable to treatment and can make a lot of difference to the quality of life of the person with Parkinson’s disease.
Unfortunately, as the disease progresses, there is a tendency for the sufferer to become confused. At the end stages of the condition there appears to be an overlap with Alzheimer’s disease and the same changes are found in the brain. For many, this intellectual impairment occurs at the same time as the disease stops being sensitive to drug therapy and the two systems’ failures are often a terminal event. For some, however, the mental deterioration appears to occur earlier and this makes their management especially difficult. There is still debate as to whether these people are subjects with Parkinson’s disease who have also developed Alzheimer’s disease or whether their chronic confusion is another manifestation of the Parkinson’s disease.

CHRONIC CONFUSION: NORMAL PRESSURE HYDROCEPHALUS
I mention this quite rare condition because it can present with a chronic confusional state, and if diagnosed early it can be treated by neurosurgery. In this condition the chambers inside the brain enlarge. The consequences are threefold: the person develops difficulty in walking (ataxia), becomes incontinent of faeces (but can also be incontinent of urine as well) and develops confusion (usually described initially as loss of short-term memory, i.e. memory for recent events). These three problems are often present in people with advanced dementia, hence the importance of recognizing the problem early. The diagnosis is made on the history, examination and findings on the CT scan, as well as other complicated tests.
If discovered early the potential for recovery is good. A shunt (a long tube) is passed from the chambers in the brain (ventricles) to the blood stream or a body cavity. In this way the excess CSF (cerebrospinal fluid) is diverted and the chambers revert to their normal size. In the past the diagnosis has been made late and the results of surgery have not been good, with very little recovery in mental function and the complications of the surgery to contend with. Early intervention can produce dramatic improvement and full reversal of all symptoms.

CHRONIC CONFUSION: BRAIN TUMOUR
In elderly people it is rare for brain tumours to present solely as a slowly progressive confusional state. Usually the confusion is accompanied by other symptoms and signs such as headache, weakness of the limbs (usually on one side only) and falls. Brain tumours are diagnosed in the same way as the collection of blood (via CT scan). Unfortunately most brain tumours in elderly people are malignant and are secondary deposits from a main tumour growing elsewhere (e.g. lung, breast etc.) Primary brain tumours of many types do occur but the prognosis for all of them is quite poor. Most tumours respond temporarily to radiotherapy or to high-dose steroids, both of which shrink the tumour and the associated swelling to stop it pressing on other vital brain structures.
Benign (non-malignant) brain tumours are quite rare. When they are detected as part of a screening procedure for chronic confusion they can be removed, depending on their size, position and on the physical state of the person concerned.
There is debate as to whether all cases of chronic confusion should have some form of brain scan (CT scan or the more recently developed MRI – magnetic resonance imaging). If all cases of chronic confusion are screened in this way the pick-up rate for brain tumours and collections of blood is low, however the general ‘cost’ to the individual and the real costs to the health service are very high when a treatable cause is missed. Accurate diagnosis would also be helped. The outcome in terms of type of care, prognosis and impact on carers between a diagnosis of dementia and that of brain secondaries is very different indeed. Current resources are severely rationed, however, and until that change it seems appropriate only to scan those people where the suspicion of a treatable or relievable cause is high, and to encourage discussion between GP, geriatrician and neurologist at every opportunity.

SNORING IN CHILDREN: CASE STUDIES ON OBSTRUCTIVE SLEEP APNOEA
After many years of recurring tonsillitis in a 7 year old girl, her parents had reached the point of desperation. This first manifested itself as snoring when the child was about 18 months old but a pediatrician assured the parents that the young girl would eventually grow into her large tonsils. Sleep related snoring and occasional episodes of tonsillitis marked the early years of her life until she was 4 years old when her mother became aware of times when the child seemed to be struggling for breath. In retrospect, judging by a description of events in the following years, the child had developed OSA, the consequences of which were to disrupt the life of parents and child for a further three years. Severity of the child’s airway obstruction no doubt reflected the status of the child’s tonsils. At best there was always a degree of snoring but a common cold or any inflammation of her tonsils would guarantee a succession of traumatic nights; traumatic for the child who would awake several times a night crying and further complicated by instances of bed-wetting and falling out of bed. It was also traumatic for the parents who were anxious about their daughter’s distress at night, not to mention the considerable disruption to their own sleep. Antibiotics probably helped to minimize the duration of these episodes but it was becoming increasingly clear that prescription of these medications was not addressing the underlying problem.
For a girl of above average ability, she was not progressing as well as could be expected and frustrated teachers would report on her tiredness and lack of application. The parents finally sought help from a pediatrician with some expertise in sleep apnoea. A hospital admission and overnight studies documented airway obstruction and oxygen desaturation consistent with OSA. Tonsils and adenoids were surgically removed a month later and the results were immediately apparent. The medical assault on high blood pressure took a step forward in the 1970s and 80s as medical practitioners embraced with enthusiasm a new group of drugs called Beta Blockers. Betnovate

When the word “protein” is mentioned most people immediately think of meat. Mistakenly, they also think that diets cannot be adequate in protein if they do not include meat. In the United States foods of high biologic value – meat, poultry, fish, eggs, milk, and cheese – provide just over three fifths of the protein in the average diet.
Legumes (navy beans, pinto beans, chick peas, soybeans, split peas, peanuts) are rich in protein, as are also nuts of many kinds. Presently legumes do not form an important part of the American diet, but their potential uses are great; they are widely used in some parts of the world.
Food technologists have developed procedures to extract the protein from plant foods such as soybeans, cottonseed, wheat, and others. The extracted protein is spun into fibers which are almost pure protein. The fibers can be formulated into textured vegetable protein foods that simulate chicken, beef, ham, bacon, sausage, tuna, and others in terms of appearance, texture, and flavor. Because of these similarities to the animal foods they are also referred to as analogs. Since these products arc intended as a replacement for meat, it is important that they provide the same variety and quantity of nutrients as contained in meat. These products arc widely used to replace meat in the diets of vegetarians.
Breads and cereals contain about 2 to 3 gm protein per serving. However, the amount of bread and cereal eaten may be sufficiently great to provide an important proportion of the total protein requirement. For example, a boy of 16 who eats eight slices of bread in a day is thus obtaining 16 to 20 gm protein from this source alone.
The average serving of vegetables contributes 1 to 3 gm protein, and fruits are even lower in protein content. This food group accounts for only a small part of the protein in typical American diets.

Valium by Hoffman La Roche was the forerunner of a large family of sedatives called the Benzodiazepines. On the face of it, the Benzodiazepines were an advance on the barbiturates, the group of drugs that they superseded because barbiturates caused respiratory and circulatory failure in overdose. A person attempting suicide with an overdose of Valium merely goes to sleep for a very long time. They wake up in an intensive care ward or the medical section of a large general hospital.
For many years the Benzodiazepines were hailed as the be all and end all in the management of anxiety and anxiety related sleep disorders. Now however the “Benzos” have fallen into disrepute. One third of persons taking Benzodiazepines become physically addicted. As time passes the same therapeutic effect requires more and more of the drug. When the drug stops withdrawal symptoms occur. Many confuse the symptoms of withdrawal with the symptoms of their anxiety disorder. They become caught in a viscous circle of ongoing habituation and inevitable withdrawal.
