INHALER TECHNIQUE FOR ASTHMATIC CHILDREN: DISCHALER
Loading the device
1. Remove the mouthpiece cover and ensure the mouthpiece is clean.
2. Hold the corners of the white tray and pull out gently until the ridged areas on the sides of the tray are fully visible.
3. Squeeze the ridged areas and pull the tray, with the wheel, out from the diskhaler body.
4. Do not remove the wheel. Place the disk on the wheel with numbers uppermost and then slide the tray back fully into the body of the diskhaler.
5. Rotate the disk by moving the tray out and in gently, until the number 8 appears in the indicator hole. As the device is used this indicator hole shows the number of doses remaining.
Using the device
1. Hold the diskhaler firmly in a level position. Lift the rear edge of the lid and raise it as far as it will go until it is fully upright, thus puncturing the blister. Some resistance will be felt as the upper and especially the lower surfaces of the blister are pierced.
2. Close the lid.
3. Keeping the inhaler level, breathe out fully.
4. Raise the diskhaler to the mouth and gently place the mouthpiece between the lips without biting. Do not cover the air inlets on either side of the mouthpiece.
5. Breathe in through your mouth as quickly and as deeply as you can.
6. Hold your breath briefly to allow the powder to reach the lungs and then remove the diskhaler from your mouth.
7. Slide the tray out and in once so that the next number appears in the indicator hole. Do no pierce the blister until immediately before inhalation.
Each disk consists of eight blisters containing medication. When the number 8 reappears in the indicator hole, all eight doses have been taken and the disk should be replaced by repeating the procedure above. Please make sure you do not throw the wheel away with the empty disk. All this sounds very complicated but in practice is simple and does have the advantage over the pressurized inhalers of not requiring coordination of breathing in while pressing the canister. The major disadvantage is that as each disk only contains eight doses it is necessary to carry spare disks and often children forget. I have also found they do tend to clog up with powder. The manufacturers do provide a brush in the rear of the diskhaler body to clean any remaining powder. This should be carried out with the tray and wheel removed from the body of the diskhaler but again in my experience children cannot be bothered to do this, and the efficiency of the device is gradually reduced.
